Could be the maf, unplug the wiring harness on the maf, start it up and see if the idle smooths out...Word of caution, the engine will not rev over 2500 rpm with the harness disconnected.
Is the throttle body clean, if not clean it with
throttle body cleaner spray, not carb cleaner.
