If it's bad, it'll generally make noises all the time, not just after starting.

They go bad from wear and tear, age, a defective part, debris, etc. Just like any other rotating part in/on the car.

Its the input shaft bearing, my car has this same problem. I've had this problem for a bit over a year now and I've yet to have any problems come from it. If you do want to replace it, I would wait until you have something else to replace in the tranny so you can kill two birds with one stone.

Yes, they're two completely different parts. One inside the trans, one outside.

TOB noise will only happen while you've got the clutch disengaged.

ISB noise will disappear when you disengage the clutch (at idle).

TOB is an easy change, just drop the trans and swap it out.

The ISB(s) are more involved, as you have to split the trans open.

I don't know if this is true on the Max or not but I had a 200SX five-speed on which I toasted the input shaft bearing (a long sad story) and I noticed it was always quietest when the car was in 4th gear because it was a 1:1 ratio and as such the gear on the input shaft simply locked to the output shaft through a synchro whereas all other gears pass power to parallel shafts and as such increase the side-loading on the input shaft bearing and, subsequently, the noise.
